    1. Human Biome Institute S.A. with its registered office in Gdańsk, Starodworska 1 (KRS no.: 0000947651, NIP: 5833384760, REGON: 385273550) is the administrator of data collected via the website: https://human-biome.com/
    2. All matters regarding the processing of your personal data, please contact the Data Protection Officer via email: rodo@human-biome.com
    3. We process the personal data you provide in the contact form available at https://old.human-biome.com/zostan-dawca/ for the following purposes:
      1. assessing the possibility of donating material in the form of intestinal microbiota with regard to name, surname, contact number, email address, PESEL number, postal address pursuant to the Article 6 (1)(a) of GDPR e. processing on the basis of your consent on data processing  and health, in particular data on: medical history, addictions, past procedures, including surgeries, acupuncture piercing or having a tattoo, or taken medicines, pursuant to the Article 9 (2)(a) of GDPR, i.e. processing based on your consent to the processing of the specific data.
      2. Establishing, investigating and enforcing claims and defending against claims in proceedings before courts and other state entities with regards to name, surname, contact number, email address, PESEL number, postal address pursuant to the Article 6 (1)(f) of GDPR, i.e. processing to implement the legitimate interest of the Administrator, involving the establishment, assertion and enforcement of claims and the defence of claims in proceedings before courts and other state authorities.
      3. handling complaints, requests and enquiries with regard to name, surname, e-mail address, telephone number, postal address, pursuant to the Article 6 (1)(f) of GDPR, i.e. processing in order to fulfil the Administrator’s legitimate interest in maintaining good relations with donors and improving the quality of services provided.
      4. creation of statistics, analyses, market research and opinion surveys with regard to name, surname, PESEL number, contact number, postal address, e-mail address on the basis of Article 6(1)(f) of GDPR, i.e. processing for the purpose of the Administrator’s legitimate interest in improving the quality of the services provided, assessing the services provided and the needs of the market.
    4. The provision of the required personal data is voluntary and is a prerequisite for you to undergo an assessment of the possibility of you donating material in the form of intestinal microbiota and for us to contact you in reference to it, i.e. if you do not provide personal data, the Administrator will not be able to assess whether it is possible for you to donate material in the form of intestinal microbiota.
    5. You may withdraw given consent for the processing of your personal data at any time. The withdrawal of your consent to the processing of your personal data does not affect the lawfulness of the processing carried out by us on the basis of your consent before its withdrawal. If you wish to withdraw your consent to the processing of your personal data, please make your request via email: rodo@human-biome.com.
    6. Your personal data will be processed until:
      1. you withdraw your consent to the processing of the personal data;
      2. until the expiry of the limitation period for claims provided for in the Civil Code or special provisions;
      3. in the case of processing for the purposes of the legitimate interest of the Administrator, until the time an objection to the processing of the personal data is filed.
    7. Recipients of your personal data may be, in particular: IT service providers, analytical laboratories, outpatient clinics, couriers and entities providing accounting, legal, analytical, records management, debt recovery services to the Administrator.
    8. Your personal data will not be transferred outside the European Economic Area or to an international organisation.
    9. Due to the processing of your personal data by Human Biome Institute S.A., you are entitled to:
      1. access to personal data, including obtaining the copy of your personal data
      2. rectification of your personal data
      3. deletion of your personal data
      4. restricting of the processing of your personal data
      5. filing an objection to the processing of your personal data
    10. If you believe that the processing of your personal data violates GDPR you have the right to file a complaint with the supervising authority, i.e. the President of the Personal Data Protection Office with its seat in Warsaw, Stawki 2.
    11. Personal data provided shall not be processed by automated means and are not subject to profiling.
